Established in 1983 with the idea to have place to support choice - Whatever the choice a woman feels is best for her life and family.



Appointment process

To schedule an appointment, contact the Center for Choice. We will ask you for some information and schedule a date and time to come in for your first appointment.  You will need to have had a positive pregnancy test prior to scheduling an appointment.  
We will ask that you bring with you to your first appointment:
-A photo ID
-Only one person with each patient (with ID)
-Please, NO Children at the clinic
-1st payment (credit card or Money order)
Visa, Mastercard, American Express and Discover

Your 1st appointment you will:
- Have an ultrasound performed
- Receive State information from the State of Ohio
- Lab work
- A one on one consultation with a doctor to have any questions answered.

Plan on being at the clinic 2-3 hours at your first appointment. 

If you are on any medications or being treated for any medical problems, please make sure and inform us when you call.  

We encourage making appointments as far in advance as possible. Our friendly staff will be happy to answer any questions you have regarding scheduling and payment arrangements.

We guarantee all medical records are kept completely confidential

Baby budget - Can I afford a baby?

 Special Instructions if you are a minor:
Ohio Law requires patient under the age of 18 to have parental consent.
This means your mother, father or legal guardian must sign a form consenting to the patient (minor)  to have an abortion.

If parental consent is not an option, minors may obtain a Judicial Bypass. This must be done through the court system and does not take very long. We can assist with this process at no additional charge. Please call our office for assistance.

It is always best to talk with a parent.  If you are just scared you may want to look at ways to bring up what will be "one of the hardest" converstations you will ever have!
